These were designed for my consultees in the beginning stages of learning EMDR therapy. Learning to navigate the different phases, prongs to the treatment plan and assessing clients' readiness for reprocessing, can be a daunting task. Consultees have reported these forms have made it much easier to keep track of the process and identify next steps.

You must have at least Adobe Reader to use the PDF versions. Make sure your Adobe Reader is up to date for the forms to work. If you have an outdated version it will not work.
