Mental Health Professionals who have purchased forms for use in their practice, may use them in their practice with their clients. Note the individual copyright requirements of each form.

You may, of course, refer people to this store to purchase their own copy of the material. Referrals to this website are greatly appreciated.

The products from the Transformative Productions store are digital in nature and cannot be returned or refunded.

Every effort has been made to give you enough information to make a choice to purchase a product or not. Make sure you carefully look over the product information before purchasing.

There are no returns or refunds of any digital products.

Downloads

After you complete your purchase, you will receive an email receipt.

The email includes a unique link for you to download the content that you have purchased, and for you to generate an invoice if they need one. 

Downloads may be in a zip file. You can do a search to find how to unzip on your device. Each operating system may have a different mechanism for unzipping zipped files.

You may have to unzip on a computer first and then transfer it to a tablet or mobile phone depending on your device.

Payments

Payments information is not stored on this website. All payments are processed by external payment services. If you choose Paypal and you do not have a Paypal account you can choose the pay by credit card option to use make your payment.

Tax Policy

Sales tax is charged depending on your location's tax laws. Tax is based on the address you use to set up your account to make a purchase.

I will be checking for support inquires. Since it is only me, I will do the best I can to get back to you in a timely manner. Sometimes my schedule is very packed, mostly when teaching. Just know I am not ignoring you.
